Perioral Dermatitis - Causes, Symptoms and Treatment

Definition:

Perioral Dermatitis "an acne-like eruption of small erythematous papules and pustules that cluster around the mouth." It is a facial rash, which is slightly scaly and bumpy. It may spread up around the nose, and occasionally the eyes while avoiding the skin adjacent to the lips.

Perioral Dermatitis is an Often triggered by higher potency topical steroids. There are no blackheads or black topped pimples on the surface of skin. The lesions can be asymptomatic or symptomatic.

It is a disease that occurs mostly in young and middle-aged women. It may also be localized or extend to involve the eyelids and adjacent glabella area of the forehead.

Causes of Perioral Dermatitis:

The important causes of perioral dermatitis are :-

  • Prolonged therapy with topical corticosteroids is main cause of this disease.
  • The use of contraceptive pill, cosmetic usage can worsen perioral dermatitis.
  • Gastrointestinal disturbances, such as malabsorption ,may also lead to the condition of perioral dermatitis.
  • Physical factors :These includes UV light, heat, and wind.
  • Certain microbiologic factors such as fusiform spirilla bacteria, candida species, and other fungi may provoke perioral dermatitis.
  • Perioral dermatitis, may be caused by toothpaste containing lauryl sulfate.
  • The prolonged use of topical steroid creams and inhaled prescription steroid sprays increases the chances to get affected by perioral dermatitis.
  • The emotional stress is also a cause of perioral dermatitis.

Symptoms of Perioral Dermatitis:

The possible symptoms of perioral dermatitis are :-

  • Uncomfortable burning sensation around the mouth may be there.
  • In the beginning, the discrete papules and vesicopustules are seen around the mouth
  • The predominant locations of lesions are the perioral area, nasolabial fold, and lateral portions of the lower eyelids.
  • The swelling and skin lesions are common in this disease.
  • The scaling and reddening of the skin is common during perioral dermatitis.
  • Itching may be there in chronic condition.
  • Perioral Dermatitis may lead to bacterial infection.
  • In some cases, bumps may be develop around the eyes and nostrils as well.

Treatments of Perioral Dermatitis:

The most effective treatments for perioral dermatitis are :-

  • Oral antibiotics, like tetracycline, doxycycline and microcycline are the most common treatment for perioral dermatitis.
  • Alcohol and hot foods should be avoided as they dilate dermal blood vessels.
  • Minimize the use of moisturizers.
  • Cosmetics should be oil-free or water based.
  • Topical creams and lotions i.e. Metronidazole cream, gel or lotion, should be applied twice a day.
  • Wash the face with warm water alone while the rash is present. When it has cleared up, wash with a non-soap bar or liquid cleanser and water.
  • Steroid creams should be avoided in all cases of this disease.
